This zone is located underneath Stormhold. You gain access by doing the A Key to the Past quest in the Stormhold Library.

The zone itself is really just one large room with several coffins. Shutting all of the coffins will spawn the Lord. The zone is basically just his lair.

There is not lockout if you die. You just spawn in the hallway above and can go right back in.

I don't know about after killing the lord.

When you enter, just to your right should be a coffin you can interact with. 'Slide' it. This will spawn a room full of 25-26 solo heroics ^^^. You need to kill each one, and close their coffin lid. Be careful though, because after about 30 seconds to a minute, the lids of killed skeletons will reopen, and you have a short time to close them again before a skeleton respawns. This zone can get very messy. After you manage to get them all shut, the same thing will happen again with the other half of the coffins and the Lord Chesgard should spawn. He's Level 30. He only dropped a Monk Adept for me
