This solo instanced dungeon is entered by clicking on a moss covered mound in The Enchanted Lands. There is also a Heroic version (entered via the same mound) named Heroic: Cavern of Tangled Weeds.

The exact location moves around and there is no guarantee at any moment that any of the mounds are up.
