This quest is obtained by examining "A lockless chest" (looks like a master chest) at +31, +128, +200 near the prison.

You have to find another chest that is similar to the first. You will find the second one at -56, +78, +171 in the Fountain Room. Now you'll need to kill the sentinels (need to be level 65+) standing along the walls in that room or in the floors near the next crossroad. These 12 rare updates, took me quite a while to finish this step. Put the 12 found medallions into the second chest.

Now the first chest will be open for 10 minutes. Head to it, open it and the quest will be completed.

Reward is a small buckler or a monk weapon - no coin.

Quest is initiated by inspecting the chest in the alcove to the right if going to the locked door at the end of the hallway past the jail, other chest is located in the room (forget name) that has Xhorroz in the corner with the Silent Sentinels lvl 65+ before the hall of rites. Fastest way i have completed the 3 quests that it offers is to get all your updates below from the 65+ sentinels for group, and then clear to the armory and go start the timer by turning in the disc's from the sentinels and it starts a 10 min timer. Run back up to armory, you have time but don't dally, clear armory so you can get thru, clear intersection and go back to chest for reward, mostly healer stuff, decent buckler for fighter classes. Hope this helps, it is made harder than it is if you do this method. Good hunting

Correction on the walkthru. Once you have placed the collected medallions in the 2nd chest, you just have to run to the first chest and open it. Quest is repeatable for different rewards

IIRC the initial chest is near the Jail room, down the hall towards the locked doors leading to the Gorg area. It appears to be a Master chest (unlike the Legendary chests for some of the other quests)

You'll find another chest like it in the Fountain room (near one of the Darslayer harvests for the Riddle of the Vault [Claymore Questline]) Activating that chest will require you to then kill lvl 65+ sentries in and around that area for 12 fairly common updates. Get those updates and then go back to the chest in the fountain room. Click the chest again and you'll have 10 minutes to return to the chest near the jail AND then return to the chest in the fountain room.

Unsure of the next step as my timer ran out.
