Next, kill the Gorehorn, Hammerhead, at 3, -6, -195 . You will receive a Hornlet Charm.
Give the Hornlet Charm to Dyrilisia. You must then talk to a junk heap called Rubbish down in the Runnyeye Chef's room at 85, -16, -233 . He is actually a scarecrow.
Now inspect a pile of books at 205, -41, -138 . This is in the tunnel past the dining hall (behind Drina the Mess Cook), near the forge on the left as you enter the room. They give some options of the book types and spawns some mobs including The Book of Baylzuthak. Kill them all.
After they die you receive a LORE/NO DROP book. Read it. You will then be told to speak to Duke Zoryll. He is an NPC Evil Eye who roams the zone.
He will ask you to kill the Thing of Shattered Iron. It is located in the room with the torturers, at 216, -16, +32 . Kill him and return to the Duke.
Lastly, you must move to the entrance of The Overlord's Throne Room (aka Runnyeye B) to complete the quest.
Hint: move back away from the door of The Overlord's Throne Room and jumping up on the ledge at about 174.16, -16.07, -221.22 . (This is before you will enter Throne Room instance, back in main Runnyeye on ledge left side of the door)
I encountered a salvaging fairy called Dyrilisia in Runnyeye Citadel. She speaks of secrets within the dungeon, secrets of great treasures undiscovered.

I'm having the same problem. I've killed the Thing of Shattered Iron four times and not received an update. I killed everything in the room. I've also sent in a /bug report. If you encounter this problem please send in a /bug and maybe they'll notice one of them and fix it.
Updating my prior post. The quest has been fixed. Killing the Thing of Shattered Iron now gives you an update. The quest can now be completed, even if the quest is grey for someone higher level.
ok u can get update for this quest..kill the chest in the throne zone and after u kill him and his guards these 3 npc's will spawn you wont be able to attack them but after 15 seconds they will attack you after you kill these zone back out into runnyeye and u will get update!
the key is still required to enter the Runnyeye's chef room. The quest is validated when you leave the instance, true but you still cannot access the throne room.
You can still finish the quest above lvl 45. When you zone into the Initial instance, you get no update but the moment you zone out of there and back into Runny Eye, it updates and clears out the quest from your journal. You do not actually need to loot the key for the update and quest completion. (as of LU#19)
Is this quest even required anymore. I read the notes for Live Update #16 and if understand it right they have removed the access quest for the epic instance. Anyone know more about this fact?
Dont bother with runnyeye b if you are 45+. There will be no chest and key drop from the duke instance to get you into the room with the lord. Bummer:(
Exact location of the Puddle of Gunk is -84,1,-117. There are 6 level 37 Gorehorn Pitmasters in a small alcove with a large eye hanging down. The puddle is directly under the Eye.
I started this quest with a group of 3, a 40 Fury, 39 Chanter and me a 42 Guardian. We had to break up right before you talk to the Duke and go into the Throne room. I came back a few days later and talked to the Duke and now I am at the Throne Room door and the quest isnt' updating?
Its taken me a while to work this out so I thought i'd share it ;) Yes, the quest finishes when you get to the door - you don't have to enter but I'd imagine you will. Once in the Overlord's Throne Room it doesn't take long to clear, if your group can cope with the rest of RE you shouldn't have any trouble. After killing everything Duke Zoryll appears again and gives you a long speech about how there's another Lord to kill, however you cant do anything else in the zone except exit back to Runnyeye. Now you need to head all the way to the other end (past the miners & torturors) to the door marked "Closed by order of Lord Balyzurth", since you've killed him you will now be able to zone through the door into the final encounter instance. Note that to have access you must have the same group as you killed Lord B. with - if you swap any members they wont have access and the group wont be allowed in. I don't know if there are any other conditions, such as not leaving RE or a timer.
as of today, 27th of march, you do not, i repeat, NOT have to actually ZONE into throne room to complete the quest.
i and 3 others in my group got quest update when we reached the door to throne room, but we couldnt enter since 2 of the group didnt have access quest completed.
and i got update BEFORE i tried to click the door, so you dont even have to try to click it for update.
in other news, you will have to click the UPPER HALF of the door to enter, the lower half is NOT clickable, might lead to some confusion or even death when you panickally try to zone out of trouble and the darn door wont highlight ;-)
Err: After examining books you need to speak to Duke. He says to kill The Thing Of Shattered Iron (in the room with torturers). Then you talk to Duke again. Only then you gain access.
did quest, got about 4% xp all and all in the end for finish. Otherwise no other reward. we nvr exited thro other door, but quest says finished..so access was for the throne room then?
I've finished this quest and done both instances. After you kill the books you are rewarded with a lore no drop book. Read it and then talk to the Duke agian. Then you are instructed to go and find the door that leads to the throne room. (forget loc) After you find it, it will update your quest and the quest is finished (kind of). The door is then clickable and you zone in. Everyone in group has to have it finished to enter. First room on left you need to go in. Its a bank room for all I can tell and ther is two mino's with a chest in between the. Kill them and the duke spawns behind you and a giant sea goblin and a bunch of minions are with him. They have this instance talk among themselves and the goblin kills the duke and then attacks you. He was easily wiped with a group of 6 34 - 36's and he drops some ok armor and a key. The key is needed once you leave the room to go up the spiral walkway to open the door. Once you go in you are now in the throne room with the lord. Some waves of monsters, agian, killed easily by my group. Then the lord attacks you. He dies pretty easily and the duke (wasn't he killed?) returns and tells you he was just the begining of the evil in runnyeye and that the lord of the eyes is the true evil and he needs to be killed. Go down the spiral walkway and zone out and then fight your way past miners, all the way to the end of the zone just before the room with the disc to zone out. There is a door that was shut by order of the lord and now he is dead so yah.... Your group can now enter this room and its another instance. It looks the same as the rest of RE with hallways and doors. Follow the hallways until you get to a hole in the floor, go through it and u'll have to kill 2 eyes in there. The hole will drop u off into a giant circular room with a eye passing around it, kill it and if your not blind you should be able to see the lord eye. He was killed pretty fast and had ok look. The duke reappears and says now that the evil is gone you can reap your reward. He asks if you want your reward now or later and once you press now you are ported back to the zone in between The Enchanted Lands and Runnyeye, oh he people at SOE are soooooo funny. :P
